Hardship opens for the failure of a third-party intermediary to submit 2025 MIPS data. Requests must be submitted through the QPP Service Center by November 1, 2026, at 8 pm ET.

QPP and PI exception applications are available. Deadline to file is December 31, 2026, at 8 pm ET.

Deadline to register for new registry-EHR integration for most registries. Check with the registry for their specific deadline.
